Participant Waiver & Release of Liability

1. Assumption of Risk

I understand that participation in basketball training, instruction, drills, scrimmages, games, and related athletic activities involves inherent risks of injury.

These risks may include, but are not limited to, falls, collisions with other participants, contact with basketballs or other equipment, running, jumping, cutting, physical contact, and other risks normally associated with basketball and athletic activity.

In consideration of my child's participation in Harry's Hoops, I knowingly and voluntarily assume, on behalf of myself and my child, the inherent risks associated with my child's participation in the program.

2. Release of Liability

In consideration of my child's participation in Harry's Hoops programs, and to the fullest extent permitted by Maryland law, I, on behalf of myself and my child, release and hold harmless:

• Harry's Hoops Basketball Training;
• Harry Auster;
• Harry's Hoops coaches, assistants, volunteers, and representatives;
• Beth Tfiloh Dahan Community School and other facilities hosting Harry's Hoops activities; and
• Their respective employees, representatives, and agents

from claims or causes of action arising out of or related to my child's participation in Harry's Hoops activities, including claims arising from ordinary negligence, to the fullest extent permitted by applicable Maryland law.

This release is not intended to waive any rights or claims that cannot legally be waived under Maryland law.

3. Participant Health and Medical Information

I certify that, to the best of my knowledge, my child is physically able to participate in basketball activities.

I agree to provide Harry's Hoops with relevant medical information, restrictions, allergies, or other health information reasonably necessary for my child's safe participation.

I understand that it is my responsibility to determine whether my child should participate if my child has a medical condition, injury, restriction, or other health concern.

4. Emergency Medical Authorization

If my child becomes injured or experiences a medical emergency during a Harry's Hoops activity, I authorize Harry's Hoops personnel to:

• Provide reasonable basic first aid;
• Contact the emergency contact provided during registration;
• Contact emergency medical services, including calling 911, when reasonably appropriate; and
• Assist in obtaining emergency medical care for my child when necessary.

I understand that Harry's Hoops personnel are not responsible for diagnosing medical conditions or determining the appropriate medical treatment beyond providing reasonable emergency assistance.

I understand that I remain responsible for medical expenses associated with care provided to my child.

5. Concussion and Safety Policies

I understand that Harry's Hoops maintains safety procedures relating to concussions, head injuries, sudden cardiac arrest, and other serious medical concerns.

If Harry's Hoops suspects that my child has sustained a concussion or significant head injury, my child may be immediately removed from participation and may be required to obtain appropriate written medical clearance before returning to basketball activities.

I understand that my separate acknowledgment of Harry's Hoops Concussion, Head Injury & Sudden Cardiac Arrest Safety information is part of the registration process.

6. Parent/Guardian Acknowledgment

By agreeing to this waiver, I confirm that:

• I am the parent or legal guardian of the participant or otherwise have legal authority to provide this consent;
• I have had an opportunity to read this Participant Waiver & Release of Liability before registering my child;
• I understand that participation in basketball involves risks of injury;
• I voluntarily permit my child to participate;
• I understand and accept the terms of this waiver; and
• I am agreeing to these terms on behalf of myself and my child to the fullest extent permitted by applicable law.
